Social Prescribing Service
Social Prescribers offer patients the time to look at ‘what matters to me’, time to explore issues and challenges and offer the opportunity for a person to make meaningful changes in their life. The aim is to empower and encourage patients to take control of their own mental health and wellbeing. The Social Prescribers will provide practical and emotional support, signpost to local services or groups, support a person in accessing appointments or services or groups. They will deliver brief interventions to educate someone to have the skills and tools to move towards independent living.

- Falls and Frailty
- Exercise and Physical Wellbeing
- Mental Wellbeing
- Home / Living Support
- Social Isolation / Loneliness
- Debt Concerns
- Signposting to services to improve Chronic Condition Management
